Registering with the Practice
All patients are allocated a named GP when registering with the practice. The named GP takes overall responsibility for a patient’s healthcare, though they may not be the GP who sees the patient on a regular basis. Patients can request to see a particular GP or other clinician, but that person may not be available at a convenient date and time. If that is the case, patients will be offered an alternative appointment with a fully qualified and appropriate clinician.
Please be aware that it may not be possible to see a clinician of your choice and at a date and time convenient to you. The practice has a wide range of clinicians who are able to treat you and we ask for your co-operation and understanding if we are unable to provide exactly what you may want.
Our priority is to provide you with the best possible care depending on your health needs and the availability of clinicians, and you may be offered an appointment with a different clinician to what you have requested. We ask that every patient shows the same level of respect and courtesy which they would like themselves, to our Patient Co-ordinators who are trying to assist you.
To register please print off and complete the two forms from our practice website or collect them from reception at either Practice site. You need to provide your NHS number and as much information as possible on the registration forms to help us ensure you get the best possible care from the start.
Once completed the forms need to be returned to the surgery together with confirmation of identity (i.e. driving licence/passport/birth certificate) and proof of address (i.e. recent utility bill, tenancy agreement or similar) where you have these items available. We will not refuse to register you at the practice if you don’t have proof of ID and address, but it helps us to ensure that we have accurate and up to date information.
If you are already on medication and/or treatment when you join the practice please ensure that we have full information about those items, so that we can ensure that you have what you need when you need it and also so that we can book a medication review with our practice pharmacist.
At the same time as you register we will register you for the Patient Online Service, unless you tell us otherwise. This will enable you to book various appointments, request repeat medication, and view your health record (including test results) from the comfort of your home. This service is available 24 hours a day.
